How To Apply
E-mail your request to: info@US-Identification.com
INCLUDE:
• The filled in, notarized form saved in a MS Word document or Adobe pdf.
• A passport-type color photo with a blue background. Please name the photo with your full name so we can readily identify it.
Once we receive your email we will respond with our address for you to mail the payment to.
• Mail two seperate money orders. One for the cost of the card for $35.00 (sent back to the applicant if the documents demanded do not comply with the requirements and/or there is suspicion of false document identification). The other to pay for the return of the card to you. $5.00 Regular mail; $10 for Priority mail; $15 for Express mail.
